AFGE Local 12 is here to help with any workplace concern or issue you are facing. Below is the link to three intake forms: the Inquiry Form, Grievance Intake Form, and Health and Safety Form. Please select the form that best fits your needs to complete. 

Once selected, please complete to the best of your knowledge, and attach any relevant documentation. The completion of these forms is to assist with tracking and information for Local 12, and is standard procedure for all bargaining unit employees, no matter the situation, dues-paying status, or circumstances. A completed intake form is not an initiation of filing a grievance, rather, a tool Local 12 uses to gather information and track concerns, and to best serve our membership.
